Temp 103.2, pulse 68.  Nourishment every 3 hours, cold sponge when temp above 102.  Fever gradually reduced to normal and below. Patient made an uninterrupted recovery.  Discharged March 9, 1908.

#380.  Mr. John Baker. D. S. S. Halderman. Residence, Earlytown, Ohio.  Admitted February 4,1908. Brakeman injured by falling from train, skull fractured at base of brain.  Never regained consciousness.  Died February 5, 1908.

Fell from moving street car.  Had slight scalp wound and in a dazed condition. Head dressed…vomited…suffered pain in head. Given morph…Calomel…removed to home march 27. Improved. #400. James Hughs. Dr. A. D. Berndt.  Residence Brewery Hollow. Admitted June 6, 1908.  Fell from street car. Was not injured.  Discharged June 7, 1908.

#401.  Mr. Jake Schmidt. Dr. F. M. Edwards. Residence Covington, Kentucky. Admitted June 7, 1908. C&O fireman was injured near South Portsmouth, Kentucky. Had fracture of skull. Necessary to trephine and remove portion of bone. Was removed home June 13. Much improved. 

#402.  Miss Merle Dins. Dr. F. H. Williams.  Residence city. Admitted June 18, 1908.  Was operated by Dr. Williams assisted by Dr. P. J. Kline. Dilation of cervix. Discharged June 22, 1908.

#403. Ed Mohr. Dr. A. D. Berndt. Residence Portsmouth, Ohio.  Admitted July 1, 1908.Right hand injured in sausage machine. Stitches taken. Hand dressed every day for awhile, then every other day, until well. Discharged July 3, 1908.

#404. Miss Lizzie Cottle. Dr. S. S. Halderman. Residence city. Admitted July 2, 1908. Prepared for operation July 4, 8:30 A. M. Anaesthetized by dr. W. W. Smith. Operated by Dr. S. S. Halderman assisted by Dr. Rardin and Berndt. Large abdominal tumor removed. Made good recovery.  Discharged August 1, 1908.

#405.  Alex Tyree. Dr. S. S. Halderman. Residence city.  Admitted July 4, 1908. Had left foot caught between cars and injured at Glen Har??, bruised considerable began walking on crutches after 10 days and left hospital the 14^th day. Discharged July 17, 1908.

#406. Charles Miller. Dr. Flint Kline. Admitted July 9, 1908. Prepared for and operated on July 10, 8:20 A. M. Anaesthetized by Dr. P. J. Kline. Operated by Dr. Flint Kline assisted by Dr. S. S. Halderman. Tumor removed from neck. Patient suffered greatly from shock. Whiskey and strych given hypodermically and saline infusion ½ pt. Patient gradually recovered and went home after 7 days. Discharged July 17, 1908.

#407. Mr. T. T. Marshall. Dr. S. S. Halderman. Residence Portsmouth, Ohio.  Admitted July 11, 1908.  Was caught between the tender of engine and platform of the Portsmouth Cold Storage Plant. Sustaining fracture of 10^th rib and severe bruising back and side, region of liver. Was able to be moved home on 12^th day.

#408.  Mrs. McManes. Dr. J. B. Rardin, city. Admitted July 15, 1908. Prepared for operation July 17. Anaesthetized by Dr. A. D. Berndt. Operated by Dr. J. B. Rardin, assisted by Dr. S. S. Halderman, J. W. Fitch.  Complete hysterectomy and appendectomy.  Appendix was adherent to fibroid tumor of uterus size of double fist. Made good recovery. Going home on August 5. Discharged August 5.  December 3, 08 greatly improved in health doing household work.

#409.  Mrs. Harry McKee.  Dr. J. S. Rardin.  Admitted July 19, 1908. Prepared for and operated July 20 by Dr. Rardin. Anaesthetized by Dr. W. G. Cheney. Retained placental and debris. Curettage and repair of lacerated perineum. Made good recovery.  Discharged August 9, 1908.

#410. Arthur Gore.  Dr. S. S. Halderman.  Residence Chester, South Carolina.  Admitted July 28, 1908. Had part of finger amputated. Discharged July30 ,  1908.

#411.  Forest Kemper. Admitted August 7, 1908.  Dr. J. S. Rardin. Operated on August 7 by Dr. Rardin, assisted by Dr. Halderman. Anaesthetized by Dr. Osborne. Separation from epiphysis of lower end of right femur, upper fragment forward, could not reduce under anesthetic. Made lateral incisions and removed one inch of lower end of deaphesis (sic) replaced applied splints later plaster dressing. Made good recovery. Walking on this leg which is thoroughly well. Discharged August 10, 1908.

#412.  Mrs. Emma Cox. Dr. J. D. Jordan. Residence Portsmouth, Ohio.  Admitted August 29, 1908. Typhoid fever. Pulse 90, temp 103 …nourishment every 3 hours. Cold sponge every 3 hrs. ..castor oil. Discharged October 7, 1908.

#413.  Mr. Daniel Phillippi. Dr. J. D. Jordan. Residence Portsmouth, Ohio.  Admitted September 4, 1908. Typhoid fever. Temp 102, pulse 96, resp. 18. Nourishment every three hours, cold sponge when temp above 102. …castor oil. Temp dropped to normal 16^th. Patient removed to boarding house September 23. Discharged September 23, 1908.

#414. Mrs. Emma Graf. Dr. J. W. Fitch. Residence Portsmouth, Ohio. Admitted September 10, 1908. Patient was prepared and operated upon at 4:00 P.M. by Dr.’s Fitch and Rardin. Anaesthetized by dr. P. J. Kline. Appendix removed which was gangrenous. Patient developed acute peritonitis and very weak heart. Patient was removed from hospital the 14^th day and made an uninterrupted recovery. Discharged September 24, 1908.

#415. Mrs. Ollie Peed. Dr. Flint Kline. Residence Portsmouth, Ohio.  Admitted Sept. 10, 1908. Typhoid Fever. Temp. 103. Pulse 84, resp. 18. Light nourishment every 3 hours. Cold sponge every 3 hours. Discharged October 8, 1908.

#416.  Grover Jackson Odel. Dr. S. S. Halderman. Residence Portsmouth, Ohio.  Admitted September 11, 1908. Had left foot amputated part of the toes right foot.  Right hand amputated and part of fingers on left hand. Was cut and bruised about the body. Bleeding from left ear.  Wounds healed rapidly was taken home September 22, 1908.

#417. Mrs. Oliver Kenedy. Dr. J. W. Fitch. Residence Portsmouth, Ohio.  Admitted September 14, 1908. Prepared for and operated upon at 2 P. M. by Dr.’s Fitch and Halderman, assisted by Dr. A. D. Berndt. Anaesthetized by dr. J. Rardin.  The operation was a cholecystostomy. The Gall bladder was thickened from inflammation, exudate and very fragile. The cavity contained two gall stones. Each about as large as a quail’s egg…patient…age 43 had been a sufferer from hepatic colic since she was 18 years old. Discharged October 12, 1908.

#418.  Miss Mary Aitken. Dr. S. S. Halderman. Residence Portsmouth, Ohio. Admitted September 24, 1908. Discharged improved October 3, 1908.

#419. Daniel Phillippi. Dr. J. D. Jordan. Residence Portsmouth, Ohio.  Admitted September 28, 1908. Typhoid fever. Temp 103, pulse 94, resp. 18. Was removed to boarding house suffered a relapse, returned to hospital.  Cold sponge every 3 hours. Light nourishment every 3 h ours…temp. gradually dropped back to normal. Discharged October 17, 1908.

#420. Mrs. Ollie Peed. Dr. D. A. Berndt. Residence city.  Admitted November 5, 1908. Gave birth to 10 pound baby boy at 5:30 A. M. November 6.  Repaired lacerated perineum. Mother and baby did very good. Removed to home in two weeks. Discharged November 19, 1908. (Extractor: Familysearch,  Ohio Births show the baby as Paul Winfred Peed. Father Ollie Winifred Peed. Mother Elsie May Cline.)

#421. Miss Edna Gist. Dr. J. S. Rardin. Residence Sciotoville.  Admitted November 18, 1908. Operated November 19, 1908 opening gall bladder and removed 43 calculi..  Fundus was firmly adherent to transverse colon which was difficultly separated. December 3, 1908 Patient has made a fine recovery left hospital …much improved. Discharged December 3, 1908.

#422. Mr. E. M. Wamsley. Dr. D. A. Berndt. Residence New Boston, Ohio.  Admitted November 18, 1908. Street car motorman received a very bad scalp wound by falling when getting off car. A number of stitches was taken to close wound. Head dressed. Removed to his home.  Discharged November 18, 1908.

#423. Miss Nettie Cook. Dr. J. W. Fitch. Resides city.  Admitted November 20, 1908. Suffering from typhoid fever. Subnormal temp 97. Pulse very rapid 120. Was very delirious given nourishment every 3 hours. Castor oil…med every 3 hrs, strych …throat in very bad condition. Died November 26, 1908.

#424. Mont Ellis. Dr. D. A. Berndt. Residence city.  Admitted November 26, 1908.  Anesthetized by Dr. P. J. Kline. Dr.’s Berndt and F. H. Williams amputated 4 toes and a part of the foot.  Was removed to his home December 3. Discharged December 3, 1908.

#425.  Mr. Robert Owens.  Dr. S. S .Halderman. Residence Dalesburg, Kentucky (Breathitt county). Admitted November 30, 1908. Anaesthetized by dr. D. A. Berndt. Operation for phimosis by Dr. Halderman…patient made rapid recovery was discharged 3^rd day.  Discharged December 2, 1908.

#426. Mrs. Robert Lyman. Dr. S. S. Halderman. Residence city.  Admitted December 6, 1908. Given anesthetic by Dr. Berndt. Operation for harelip. Dr. S. S. Halderman assisted by Dr. Moore. Patient did very nicely.  Removed home next morning. Discharged December 7, 1908.

#427. Mr. Thomas McCann. Dr. S. S. Halderman. Residence Rardin, Ohio.  Admitted December 9, 1908. Given Anesthetic by dr. D. A. Berndt. Operation for phimosis by Dr. Halderman. Patient recovered rapidly. Patient recovered rapidly. Discharged December 10, 1908.

#428. Mr. William Dietz. Dr. D. A. Berndt. Resides city.  Admitted December 23, 1908.  (No further treatment or discharge or any entry after admitted.)

There are several paper items tucked inside the Park Hospital ledger.  Among the items included is a photograph of a very ill man.  The picture was labeled by Dr. H. G. Halderman. “Jack Lavour age 45   .  Admitted October 24, died Nov 2, 1902. Confluent variola. 8^th day of eruption..  Taken Nov. 1, 1902. He was never vaccinated.  Since this was just prior to the 1903 ledger this extractor wonders if he might have been the first person admitted to Park Hospital.

Several receipts are included in this grouping. The last is D. A. Berndt, MD, Eighth and Gay Street payable to Dr. S. S. Halderman dated July 1, 1910 for professional services of anesthetics from June 1909 with a list of the patients which I included here for researchers:

Crushed leg. Injured at the Portsmouth Steel Plant.  Temp on admission 101.8, pulse 106.  Treatment quinine…dressing changed each day.  Patient gradually improved left hospital the 24^th day. Discharged March 21, 1907.

#316.  Mr. M. E. Burk.  Dr. S. S. H alderman.  Residence city, Washington Hotel. Admitted March 12, 1907.  Diagnosis Medical treatment (as written). Discharged March 13, 1907.

#317.  Mrs. Charles Taylor.  Dr. E. M. Foster. Residence Portsmouth, Ohio.  Admitted March 15, 1907.  Was prepared for operation March 16 10:50 anaesthetized by Dr. L. G. Lock. Operated by Dr. E. M. Foster, assisted by Dr. Harry Schirrman. Abdominal incision made, Salfingo oophorectomy (removal of ovary). Patient rallied well …no nausea, made an uneventful recovery.  Went home the 14^th day. Discharged March 29, 1907.

#318. Mr. Clarence Cisna. Dr. J. B. Jordan. Residence Portsmouth, Ohio.  Admitted March 17, 1907. Typhoid Fever. Temp. 103, pulse 120…fever gradually reduced to normal and below. Patient made an uninterrupted recovery. Discharged on 23 day. Discharged April 8, 1907.

#319. Mr. Green Kinner. Dr. Jos. S. Rardin. Residence Buchanon, Kentucky.  Typhoid fever. Temp on admission 104.4, pulse 130 cold sponge when temp above 102…gradually reduced to normal. Patient went home on 24^th day. Discharged April 8, 1907.  (Extractor notation: This is possibly Green Kinner wife Susan Ross.  Green Kinner lived until 1980 buried in Ross Moore Cem., Lawrence County, Kentucky.)

#320.  Mr. R. E. Hauser.  Dr’s. Halderman & Berndt. Residence Portsmouth, Ohio. Admitted March 23, 1907. Engineer hurt in a wreck. Badly burned and scalded about the head, hands and body. Compound comminuted fracture of left leg between knee and ankle. After entering hospital wounds dressed. Patient grew rapidly weaker. Died 2:15 P. M.

#321.  Mr. Mike Friel. Dr. Fulton. Residence Portsmouth, Ohio.  Admitted March 23, 1907.  Temp. 103, pulse 110 on admission. (Diagnosis not cited).  Discharged April 17, 1907.

#322.  Mr. J. E. Bevins.  Dr. S. S. Halderman.  Residence not cited.  Admitted March 25, 1907.  Fireman injured by falling from train at Kenova (WV). Badly bruised about the body. Temp 103.8, pulse 88. Given Calomel in broken doses. Patient improved went home on ninth day.  Discharged April 10, 1907. (Many entries list calomel which is a mercury based purgative and was used to kill bacteria.  Early records show it used for George Washington just prior to his death.  It was still listed as a medicine in the 1960’s before widely accepted as a poison.)

#242. January 3, 1906.  Mrs. Jacob Jones. Dr. Brendt.  Residence 17 East Front St., Portsmouth, Ohio. Widow, age 89.  Admitted to hospital Jan3, 1906. Was in hospital two days previous to operation.  Anesthetized Saturday, 8:15 A. M. by Dr. Kline, operated upon by Dr. Berndt, assisted by Dr’s. Halderman and Allard.  Two rectal fistulas. These were opened, curetted and sutured. …Iodoform gauze applied as dressing…Not much nausea…Recovery complete and satisfactory. Removed to her home Jan. 17^th, 1906. 

#243.  January 3, 1906.  Mr. J. C. Glover. Admitted to hospital Room 5. Dr’s. Kline and Halderman.   Residence Biggs House, Portsmouth. Single age 43. Patient unconscious when brought in ambulance to hospital.  Was found in semiconscious state at 3 P. M. in room at Biggs Hotel. Had not been very well in few days previous.  Dr.’s Kline and Halderman were called, suggesting patients removal to hospital.  Convulsive movements of limbs; vomiting. Temp 99…Rallied second day.  Convulsions less frequent and severe. Drank freely of water and milk.  Began taking light diet in four days, gradually to solids.  …was not fully recovered when left hospital.  Returned to Biggs House. Discharged Jan. 30, 1906.

#244. January 18, 1906.Mrs. Chas. Martin. Admitted to hospital room no. 2. Dr. Halderman. Residence 496 Robinson Ave., Portmouth, Ohio. Married age 28. Patient brought to hospital in Dr.’s carriage. Prepared for operation. Anesthetized by Dr. Rardin, operated by Dr. Halderman, assisted by Dr.s Fitch and Edward’s. Uterus curetted and packed with gauze. Returned to room in good condition. Passed comfortable night.  Temp. 103.6…Patient condition much improved (3 days)…appetite good. Sleeps well. Discharged January 26, 1906.  Returned home in Dr.’s carriage.

#245. January 20, 1906.  Miss Eunice Vernon. Admitted to room no. 3.  Resides Portsmouth, Ohio. Dr. Halderman.  Prepared for operation.  Operated by Dr. Halderman, assisted by Dr. Rardin. Cancer taken from lower jaw. Patient rallied nicely….remained as patient 5 days.  Discharged but remained in hospital as nurse.

#246.  January 22,1906.  Miss Alice Arthur. Admitted to room no. 4. Dr. Halderman.  Residence 2.40 E 12 St. (as written), Portsmouth, Ohio.  Anesthetized by Dr.Brendt. Operated by Dr.’s Halderman (as written assume both S. S. and Henri G. Halderman). Brought to hospital in ambulance and put to bed at once and prepared for operation.  Bullet wound in left hip.  Bullet entered lower intestine…perforations repaired…patient ralled from shock condition. Critical from first…morphine…often as necessary…strych…calomel…patient grew rapidly worse. Died 12:15 P. M. January 25, 1906.

#247. January 26, 1906.  Mrs. Myrtle Selb.  Admitted to hospital room no. 7.                             Dr.Flint Kline. Patient prepared for operation. …Anesthetized by dr. P. J. Kline. Operated by Dr. Flint Kline, assisted by Dr.’s Halderman and Robe.  Uterus corrected. Patient returned to room in good condition…Returned home in 7 days.  Condition very much improved. January 31, 1906.

#248.  February 2, 1906.  Mr. Harry A. Livingston. Admitted to room 5.  Dr. Halderman. Residence Hamden, Jackson Co., Ohio.  Brought to hospital in ambulance in semiconscious condition. Pulse not perceptible in arm.  Examined by dr. Halderman. Left leg crushed just above knee. Complained of severe pain in abdomen. Gave hypo. of morph. And strych. No reaction. Whiskey…Patient did not rally. Died 8:30 A. M.

#249.  February 2, 1906.  Mr. Chas. Mortz.  Admitted to room 4.  Dr. Williams. Residence E. Ninth Street, Portsmouth, Ohio. Patient was brought to hospital on street car. Had left hand crushed in Portsmouth steel works.  First finger and thumb were amputated. Remained in hospital overnight.  Taken to his home to be treated Feb. 3, 1906.  (Extractor notation: street car ran in front of hospital and Tracy Park.)

#250.  February 2, 1906. Mrs. Florence Hamm. Admitted to room no. 3.  Dr. Micklethwait.  Residence Kenova, West Virginia. Brought to hospital in ambulance. Temp. 102.2, pulse very fast and irregular. Vomited at intervals for 4 days.  Some symptoms of jaundice. …light diet, slept well…continued to improve and was removed to boarding house in bac. Feb 17, 1906.

Admitted to room 6.  Temp. 103.4, pulse 100.  Nourishment given every   2 hours…bath…patients temperature gradually reduced…March 5, 1906 patient sitting up in chair and moving around the room.  Discharged March 7, 1906.

#252. Mr. Patsy Grazgrine. Dr. Smith Residence “Italian Camp.” Patient admitted to hospital February 22, 1906 3:30 P. M. Room no. 3. Temp 100.6 and pulse 82.  …patient temp gradually reduced and general condition improved at the end of four days….sleeps well…gaining strength. Discharged March 8, 1906.

#253. Ross Huston, age 13. Dr. Rardin. Patient entered hospital room no. 4. Suffering from a lacerated wound of second digit on right foot resulting from the accidental discharge of a shot gun.  Was anaesthetized by Dr. Halderman and toe amputated by Dr. Rardin. Patient returned to room in good condition, was somewhat nauseated but slept the greater part of the night. Dressing done by Dr. Rardin each day. Wound healing nicely.  Patient discharged on 14^th day.  Discharged March 10, 1906.

Patient admitted to the hospital room 6, March 9, 1906.  …put on …diet with raw egg and sherry…absolute rest in bed with very little improvement in condition at the end of two days.  Operation decided upon. Patient prepared and operated on third day after admission.  Anaesthetized by Dr. S. S. Halderman, operated by Dr. Edwards, assisted by Dr. Rardin.  Abdominal incision was made, adhesives broken up and appendix removed…patient returned to room in good condition and very little nauseated….Patient made a rapid recovery…patient removed from hospital in ambulance Tuesday March 20, 1906.

Dr. Halderman.  Residence Sciotoville, Ohio.  Patient admitted to hospital room no. 3.  March, 12, 1906.  Temp. 99, pulse 94.  Patient was anaesthetized by Dr. McCann and aspirated by Dr. Halderman.  No fluid obtained.  Patient remained in hospital two days under observation of the doctors.  Discharged March 14, 1906.

#257. Mr. George Rishforth. Dr. F. M. Edwards. Residence Covington, Kentucky. Admitted to hospital room no. 6, March 31, 1906.  Suffering from injuries received in a C&O. R.R. wreck which consisted of two scalp wounds and sprained back and side.  Patient made a rapid recovery being dismissed on the sixth day.

#258. Mr. Joseph Hand. Dr. F. Edwards. Residence Covington, Kentucky. Patient admitted to hospital suffering from internal injuries received in a C&O R.R. wreck, March 30, 1906. Was in a comatose condition when admitted and died two hours after.  Died March 31, 1906.

#259. Mr. Kendall Monroe. Dr. Williams and Micklethwait.  Residence 314 Twelfth St., City.  Patient admitted to hospital March 31, 1906 suffering from a bullet wound in abdomen also two flesh wounds in left side and on the outer side of left hand. Was immediately anaesthetized by Dr. Rardin and operated by Dr. F. M. Williams assisted by Dr. Flint Kline. Incision line and …where bullet entered. It was found that the bullet ranging downward pierced the intestines in several places which were repaired. Patient rallied well …made an uneventful recovery and was removed to house on the 20^th day. Discharged April 19, 1906.

#260. Mr. Sherman Lyon. Dr. S. S. Halderman. Residence Portsmouth, Ohio. Patient admitted to hospital April 12, 1906. Fractured skull.  Absolute rest in bed in darkened room…(list of medications) patient gradually improved and was discharged on the 8 day.

Admitted April 16, 1906.  Suffering from extensive burns received by being accidentally caught in a steel plate mill at the Portsmouth Steel Plant. Burns extending from left shoulder to heels…intensity from first to third degree…patient made a tedious recovery remaining in hospital 10 days.  Discharged June 25, 1906.

#276. Miss Tillie Howe. Dr. S. S. Halderman. Residence South Portsmouth, Kentucky. Patient admitted June 22, 1906. Was prepared for operation June 23 8 A. M. Anaesthetized by Dr. Smith operated by Dr. H alderman assisted by Dr.’s Rarden and Brendt. Tumor removed from cervix and curettage. Iodoform gauze …did well for a few days then developed a temp ranging from 101.5 to 104.4. July 1 patent had two severe chills…Patient complaining of a great deal of pain. Discharged improved July 15, 1906.

Had been ill about a week before entering hospital very much nauseated, unable to retain anything on stomach.  Given a little cracked ice…patient rested well during night…(description of enema and medications) Patient doing well. Discharged on  9^th day.  Discharged July 23, 1906.

Discharged October 5, 1906.

#288. Colonel Bazler. Dr. D. A. Berndt. Residence city. Patient struck by B*O engine while out flagging. Asleep on track. Was removed to hospital and prepared for immediate operation. Was anaesthetized by Dr. Halderman. Operated by Dr. Berndt assisted by Dr. Allard. Amputation of right arm and shoulder joint. Fracture of skull in right occipital region. Patient did not rally.  Death resulting  from shock. Died September 30, 1906.

#291. Mr. Charles Weeks. Dr. S. S. Halderman. Residence Portsmouth, Ohio.  Admitted October 11, 1906. N&W Rwy. Brakeman. Injured by falling from moving train. Compound fracture of left arm and injury in axillary space.  In hospital 32 days. Discharged November 10, 1906.

Compound fracture of left leg and flesh wound on right knee.  …put in splints for three days then put in a plaster cast after which patient allowed to sit up in chair and be about room on crutches. Dismissed form hospital on 9^th day. Discharged October 31, 1906.

#295. Mrs. Bessie Floyd. Colored.  Dr. S. S. Halderman. Residence 168 E. 10^th Street, city.  Admitted December 2, 1906. Temp. 99.4, pulse 110. Patient suffering from pain in right side, great tenderness over entire abdomen. Pain controlled by opiates.  Condition improved …still under observation and condition still improving but would not consent to operation.  Discharged December 16, 1906.

#296. Mrs. W. D.Tremper. Dr. S. Halderman. Residence Portsmouth, Ohio. Admitted December 6, 1906. Was anaesthetized by Dr. Rardin, operated by Dr. Halderman assisted by Dr. Margaret Fulton. The urethra dilated and a small tumor removed. Patient very nervous and excited from anaesthetic, very much nauseated.  But had a comfortable night and made an uneventful recovery.

#299 Mr. H. C. Duncan. Dr. S.S. Halderman. Residence Portsmouth, Ohio.  Admitted Dec. 24, 1906. Was injured in street car accident. Cut and bruised about the face and head. Discharged Janauary 2, 1907.
